Instruction of download & print:-
1. Right click "Save Image As", suggest to save it on your Desktop
2. Print page 1 & 2 out as either "full page fax print" or "full page photo print"
Just fill your particulars, prepare the payment & then send them to us by either mail or drop the form at the information counter of PPUM.
